Two South Cook nursing homes hit with health violations

Two nursing homes in South Cook County have been cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the second quarter of 2017.

The report, released on Monday, lists the nursing homes in the state with AA or A violations, which might have caused the death of a resident or could cause death or serious injury to a resident, respectively. Both of the facilities received A violations and $25,000 fines.

Midway Neurological/Rehab Center, a 404-bed skilled care facility at 8540 S. Harlem Ave., Bridgeview, was cited with an “A” violation and fined $25,000 for an incident where a resident entered a restricted area and fell five stories down a laundry chute, sustaining multiple broken bones and requiring hospitalization and surgery. The facility waived its right to a hearing and paid $25,000.


Lydia Healthcare, located at 13901 South Lydia, Robbins,was cited with an “A” violation and fined $25,000 for failing to prevent a resident from ingesting non-edible objects. The 412-bed intermediate care facility waived its right to a hearing and paid $16,250.
