Holy Family Villa, a 129-bed skilled care center in Palos Park, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Holy Family Villa was cited with an A violation in December and fined $25,000.
