Location of death: 800 W. Oakton St. Room 1317, Arlington Heights

Died from:

-Primary cause: pneumonia, COVID-19

-Secondary cause: hypertensive and valvular heart disease, atrial fibrillation, rectal cancer, b cell lymphoma
