Diane M. Van Horn, nee Kelly, aged 79, of Bridgeview, Illinois, passed away surrounded by her loving family. She is survived by her husband of 59 years, Peter Van Horn, and her son Peter Jr. Diane was a beloved grandmother to Brandon, Michael, Kayle, Carmella, Madison, and also shared a special bond with Justin, Samantha, and Zosia. She was a treasured great-grandmother to Grayson, Easton, Maddux, Colton, Zayden, Ivy, and Adalina. Diane was the dear sister of Patrick Kelly and loving aunt to many nieces and nephews.

Diane was known for her humor, warmth, and timeless style, carrying herself with a youthful spirit. She enjoyed shopping, cooking, and hosting family gatherings, creating lasting memories with her grandchildren. Her unconditional love, kindness, and selflessness will be forever cherished by all who knew her.

A visitation to share memories of Diane will be held on Friday, September 19th at Hann Funeral Home from 3-8 PM. Services will continue on Saturday, September 20th from 9:00 a.m. until the time of the Prayer Service at 12:30 p.m., followed by interment at Resurrection Catholic Cemetery.
